About Your Visit
Your initial appointment will consist of a physical exam, x-rays, consultation explaining your diagnosis and the proposed treatment plan.
What to bring to your first visit:
- X-rays
- MRI films, disc and reports
- Past history; surgery/operative reports and pictures if available.
Please also remember to bring:
- Picture ID
- Insurance Card
- Complete list of medications
- List of allergies
- Any Worker’s Compensation or Auto Accident information. This should include; Name of Insurance Co., claim number and agents name as well as contact number.
